I hereby request the Council of the British Association of Oral Surgeons to recommend my name for election as a Member / Associate of the association at the next Annual General Meeting. If elected, I promise to abide by the Rules and Regulations of the Association as set out in the Memorandum and Articles of the Association, or such amendments to the same which from time to time may be approved in accordance with the Articles.
